The Opportunity:
Our client is seeking a compassionate and experienced Nurse Case Manager to join their team in Windsor, Ontario. This position plays an important role in supporting clients and their families throughout the home care journey, from initial assessment and service coordination through ongoing follow-up and quality assurance.

What You’ll Be Responsible For:
- Conducting comprehensive client assessments to identify home care needs and develop appropriate service plans.
- Managing client cases from initial assessment through ongoing service delivery, follow-up, and changes in care needs.
- Building trusted relationships with clients and families while ensuring expectations, concerns, and service issues are addressed promptly.
- Coordinating with caregivers and internal team members to support appropriate client-caregiver matching and continuity of care.
- Performing scheduled client visits and spot checks to monitor safety, satisfaction, service quality, and changes in client status.
- Updating client care plans, records, assessment information, and case notes accurately and within established timelines.
- Communicating changes in client needs with families, caregivers, healthcare professionals, and other members of the care team.
- Collaborating with community healthcare partners, retirement and long-term care professionals, and referral sources to support client care.
- Following up with prospective clients and families after assessments to answer questions and support informed care decisions.
- Promoting available home care services to prospective clients, families, healthcare professionals, and community referral partners.
- Assisting with third-party billing, client and employee records, administrative documentation, and other operational requirements as needed.
- Participating in community health and wellness events, information sessions, and other outreach initiatives.
- Maintaining strict confidentiality of all client information and complying with applicable legislation, professional standards, and organizational policies.
The Ideal Candidate:
- Bachelor of Nursing degree or Registered Practical Nurse (RPN) diploma, combined with supervisory experience.
- Current registration and licensure in good standing with the College of Nurses of Ontario.
- 2 – 3+ years of nursing service delivery experience, ideally supporting older adults and/or families.
- Demonstrated understanding of community support services, healthcare resources, and care options available to clients and families.
- Proven ability to advocate for clients, solve problems, organize competing priorities, and achieve positive outcomes.
- Strong assessment, care planning, documentation, and case management capabilities.
- Excellent interpersonal and communication skills with the ability to establish trust with clients, families, caregivers, healthcare professionals, and community partners.
- Strong attention to detail and ability to maintain accurate and confidential client documentation.
- Valid driver’s licence in good standing and ability to travel to client homes and community locations as required.
- Compassionate, client-focused approach with a commitment to promoting independence, dignity, safety, and well-being.
- Experience working in home care, community nursing, senior care, retirement living, or a related healthcare environment.
